1. Full name as it should appear on the certificate
2. Year approved to sit for conversions
3. Year, examination session, and candidate number for EACH module passed

Please note that upon collection you will be required to pay USD 35 per certificate: two certificates for those with exemptions (one completion and one exemption certificate) and one certificate for those who wrote 8 subjects.
